The Tax Forms Info section in the questionnaire is where foreign nationals will provide annual information relevant to their tax obligations in the United States.
Access Tax Forms Info
1. Login to your Calculus account.
2. Head to the Users tab > All Accounts and click anywhere on a user’s record.
3. A side panel will appear right side of the screen.
4. Under Account Details, click on the Edit button.
This will bring you to the Foreign National Questionnaire.
5. On the left side Progress Bar, select Tax Forms Info.
6. Foreign Nationals may select the type of income they are receiving.
Info: Institutions may pre-select the income types and Individuals can choose to deselect the codes that not relevant to them.
Info: Institutions can pre-select the income types that cannot be edited by Individuals.

7. The Tax Forms Info page must be completed annually by individuals to produce Form 8233, "Exemption From Withholding on Compensation for Independent (and Certain Dependent) Personal Services of a Nonresident Alien Individual." This requirement ensures that any changes affecting residency status, treaty eligibility, or employment conditions are promptly updated.
8. Individuals should update this section as soon as circumstances change, such as starting a new job, changes in wage amounts, marital status adjustments, or any other factors influencing tax liability or reporting requirements.
9. A foreign national who is a resident of a tax treaty country and is employed on campus,
will be generally asked to provide approximate amounts of expected wages/compensation for the calendar year. Individuals will also be asked to provide a short description of the personal services they've performed for your organization. Both of these fields are related to Form 8233.
Important: Any amounts in this page are not going to be populated on the 1042-S Forms.
